Notice of Privacy Practices (HIPAA)

Effective Date: April 16, 2025

This Notice of Privacy Practices (“Notice”) describes how Ample Health & Wellness may use and disclose your protected health information (PHI) for health services, and your rights regarding that information. This Notice applies to our coaching, consulting, and wellness services. For information on eCommerce purchases and affiliate sales, please see our separate eCommerce Terms & Policies.

We are required by law (HIPAA) to maintain the privacy of your PHI, provide you with this Notice, and abide by its terms. We may change this Notice at any time; revised versions will be available upon request and on our website.


How We May Use or Disclose PHI

Treatment: To coordinate your care with other providers, trainees, therapists, medical staff, pharmacies, or labs.

Payment: To obtain payment or pre-authorization from insurance companies or third-party payers.

Health Care Operations: For quality assessment, staff training, business operations, and appointment reminders via phone, text, or email.

Business Associates: Third-party service providers (e.g., billing companies) under written contracts that protect PHI.

Marketing & Sale of PHI: We will not use or disclose your PHI for marketing purposes or sell your PHI without your written authorization, except for limited cases permitted by law (e.g., face-to-face communication or nominal promotional gifts).

Breach Notification: We will notify you without unreasonable delay if a breach of unsecured PHI occurs.

Other Uses & Disclosures: As required or permitted by law, including:

  • Public health reporting

  • FDA oversight

  • Health oversight agencies (audits, investigations)

  • Workers’ compensation

  • Lawsuits and legal actions

  • Law enforcement with proper legal authority

  • Organ/tissue donation (if applicable)

We will not use or disclose PHI beyond these purposes without your written authorization, which you may revoke at any time (except to the extent already acted upon).


Your Rights Regarding PHI

Access: You may request to inspect or obtain a copy of your PHI. We will respond within 30 days (with one 30-day extension if necessary). Reasonable cost-based fees may apply.

Amendment: You may request amendments to your PHI. We will act within 60 days (with one 30-day extension). If denied, we will provide a written explanation and your right to submit a statement of disagreement.

Accounting of Disclosures: You may request a list of disclosures of PHI (excluding those for treatment, payment, and operations) for the prior six (6) years.

Restrictions: You may request restrictions on use/disclosure. We are not required to agree, except we must honor requests to restrict disclosure to a health plan if you pay in full out-of-pocket.

Confidential Communications: You may request alternative means or locations for communications, and we will accommodate reasonable requests.

Paper Copy: You may request a paper copy of this Notice at any time.

Complaints: If you believe your privacy rights have been violated, you may:

Safeguards for Your Information

We use administrative, physical, and technical safeguards to protect electronic PHI (ePHI) in compliance with the HIPAA Security Rule.
